Book a Medical Appointment

Call us at 613-233-4697 to book your appointment. Our medical receptionist will ask you to provide key information and set-up an appointment with your doctor or nurse practitioner.

Become a Patient here

When accepting new clients from Centretown, the Glebe and Old Ottawa South we will advertise in the community and on our website.

We encourage you to register with Health Care Connect a service of the Ontario Government to help you find a family doctor or nurse practitioner. 

Refill my Medications

Call your pharmacy and ask to refill your prescription.  They will take care of contacting us to authorize a refill of your medication.

Be sure to contact your pharmacy before you run out of medication.  It can take up to two working days for us to authorize them to provide refills.

Find out the Results of a Test

Call us at 613-233-4697 to make your request with the medical receptionist. Arrangements will be made and someone from our Centre will be in touch to deliver your results.

Book an Appointment with a Lawyer

At our Centre, Legal Aid Services provides free legal advice to those who don’t have the means to pay for it. On a first-come, first served basis, clients can speak to a lawyer from 15 – 30 minutes. Check our calendar for dates.

Appointments are not accepted and services are offered in English and French. Those who don’t speak English or French can access these services by bringing along an interpreter.

Qualifying for Legal Aid

Legal Aid is available to those who are currently:

  • Ontario Works recipients
  • Ontario Student Loan recipients
  • Ontario Disability Support Program recipients
  • Old Age Security, Gains, Income Security Program recipients
  • On a low income (questions about your income will take place at the start the session)
  • In an institution

Legal Services at our Centre

At our Centre, qualifying clients can access

  • Legal Advice
  • Advice on how to write documents
  • Information about what happens in court
  • Suggestions and information about community and government agencies

Legal Aid can also help by explaining legal documents, including statements of claim, statements of defence, and many others.

Services offered also include notarizing documents and administering oaths to swear to written statements. Clients are required to bring such statements unsigned and written out completely.

Join the LESA Program

Joining the LESA Program is just a phone call away

To join, clients must be residents of Ottawa-Carleton who:

  • are 55 years of age or older
  • live independently
  • use alcohol, drugs, and/or gambling in a way that is presumed or confirmed to be hazardous
  • express a desire to improve their health and well-being

Abstinence from alcohol, drugs, or gambling is not a criteria for admission. However, abstinence from alcohol, gambling, and the proper use of medications are strongly encouraged and usually become a goal of treatment.

Referrals to LESA are typically completed over the phone and may take place in either French or English. We invite the individual to make the call.

Health professionals and family members are also welcome to refer clients who have given them consent to do so. In instances of residential or hospital settings, the referral agent can request to have a counsellor screen and connect with the client in person before discharge

Recognize the problems related with alcohol, drugs, or gambling

The Team offers a variety of presentations and workshops specifically designed for professionals who are regularly in contact with older adults. Participants will learn to recognize signs or problems related with alcohol, drugs, or gambling and how to effectively intervene with older adults

Professionals seeking advice related to older adults’ alcohol use, drug use, or gambling are invited to contact the LESA Team. We’ll gladly answer your questions and provide helpful advice. We’re always keen to discuss how to broach this difficult topic with clients and recommend ways to engage them in an appropriate treatment program.
