Trans Health Program On-going Funding

We are pleased to share the news that CCHC was granted on-going funding for its Trans Health Clinic serving Trans and gender diverse people. This sustainable funding will allow us to serve more people and enhance our services. 

 

The consultation

In order to meet the needs of the community, CCHC, with the support of the Champlain LHIN and the Champlain Regional Planning Table for Trans, Two Spirit, Intersex and Gender Diverse Health, Mental Health and Social Services, initiated a review of hormone initiation services in the Champlain region to improve access and to better integrate this service into mainstream primary care.

To ensure transparency, CCHC hired independent contractors to do consultations with community members and health providers.   

As a result, 44 Trans and gender diverse people and 30 health professionals were consulted through surveys, interviews and a workshop.

The review process identified three populations that were being underserved in the Champlain region: youth, francophone and rural populations.

 

The main outcome of the consultations

The consultations identified the opportunity to leverage CCHC expertise to:

  • increase its capacities to offer hormone initiation services to trans and gender diverse people
  • build capacity of primary care providers throughout the Champlain region.

 

Responding better to the needs of Trans and gender diverse communities in the Champlain region

The on-going and sustainable funding will allow us to address concerns raised by the Trans and gender diverse community in the following ways:

  • Support more people: this funding will allow us to increase our clinic days from one day a week to two days a week (starting Fall 2019). With this funding we are also committed to looking at how we address the needs of Trans youth 17 and older. 
     
  • Enhance communication with community and stakeholders: with confirmed funding, we are committed to ensuring clearer and more accessible information on the program as well as collaboration with the community and other key stakeholders to inform our development. 
     
  • Extend our expertise: we share our expertise to continue to build capacity amongst primary care providers throughout the Champlain region, including in rural areas. Special attention will be given to providers who offer services to Francophone communities.

Individuals who call our central referral number will be offered a 60-90 minute intake meeting with a counsellor to explore gender transition goals, service needs and options.

During the intake meeting with a counsellor, the following options and service pathways will be discussed:

  • Referral to a medical provider for hormone therapy
  • Connection to transition supports
  • Referral to counselling supports if needed
  • Assistance in linking to a primary care provider if needed
