About the Role
The Preventive Care Program aims to improve preventive care for equity deserving and undeserved populations and ultimately reduce chronic disease incidence and complications by facilitating access to primary care, preventive programs/services, promoting healthy behaviours and screening and assessing broader impacts of holistic preventive care on wellbeing.
The program focuses on modifiable factors of health (e.g., behaviour change) and acknowledges that systemic and structural barriers (e.g., racism, discrimination, colonial policies) impact participants’ opportunities and abilities to accomplish their preventive goals and considers chronic disease risk factors (e.g., tobacco use, alcohol consumption, physical inactivity, unhealthy eating), overall health and wellbeing (e.g., physical, mental, social needs of an individual), social determinants of health (e.g., income, food security, housing, employment and working conditions, education) as well as structural and systemic barriers (e.g., discrimination, disparities in access) impact individuals’ opportunity for health and wellbeing
The Prevention Specialist is responsible for providing education about chronic disease risk factors and how to prevent chronic disease (e.g., physical activity, healthy eating guidelines). The Prevention Specialist will work with equity-deserving participants to provide health education about chronic disease risk factors, co-develop preventive care goal(s) and a Preventive Care Plan to help them achieve their goal(
s), and connect participants to primary care and other programs and services based on their preventive care goal(s), including referrals or connections to community and social services.
The Prevention Specialist works collaboratively with key CCHC staff / teams as well as allied community partners to provide the various supports on a group and individual basis.
Requirements
Bachelor degree in health, social work, or related discipline or equivalent combination of training and experience
Minimum of three (3) years of relevant experience in community health care, health education and program development
Experience working with high priority and at-risk communities, with knowledge of relevant services and community supports in Ottawa is strongly preferred.
Experience implementing motivational interviewing and supporting behaviour changes through SMART goal setting.
Experience in providing culturally responsive health education using rapport building, trauma-informed, harm-reduction approaches based on community values, attitudes and beliefs.
Demonstrated ability to work with equity deserving populations within a trauma-informed, anti-racist and anti-oppressive lens.
Ability to work well under pressure in a fast-paced environment and be adaptable to the changing needs of the organization.
Detail-oriented with ability to multi-task, organize, prioritize, and meet deadlines.
Excellent interpersonal and collaboration skills with a proven ability to successfully deal with complex issues working within the context of cross-functional teams.
Experience leading and facilitating groups utilizing adult learning techniques.
Experience with conflict resolution skills.
Experience working from a health equity perspective.
Knowledge of Health Promotion principles.
Experience and accuracy using database applications (i.e. Electronic Health Records) and proficient with Microsoft applications (Word, Outlook, Excel, PowerPoint & Publisher).
Excellent verbal and written communication skills in English and French.
Ability to work well independently and as part of multi-disciplinary team.
Ability to work some evenings and weekends.
