About the Role

The Registered Nurse (RN) – Urban Health Outreach works with homeless, marginally housed, street affected and newcomer to Canada clients to help them achieve their desired health goals. The incumbent, as part of a multidisciplinary team, provides client-centred primary care within a care modality, which respects client needs and choices, and within a health care framework, which emphasizes health promotion and education, disease prevention and on-going client education. The Urban Health Outreach RN works in collaboration with community organizations serving the homeless and newcomer populations.

Requirements

  • Bachelor degree in Nursing or equivalent combination of education and experience.

  • Registered and in good standing with the College of Nurses of Ontario.

  • Eligible for membership with the Registered Nurses’ Association of Ontario.

  • Minimum 2-3 years’ experience in a community and/or public health setting.

  • Work experience with street affected underserved individuals with mental health and

  • Knowledge of the health needs of people experiencing homelessness and newcomer

  • Skills and experience working with diverse populations.

  • Demonstrated respect and value for the diversity of the community and individuals.

  • Ability to work independently and within a multi-disciplinary team.

  • Strong clinical assessment skills.

  • Good problem solving skills.

  • Strong verbal communication skills and experience, including de-escalation strategies and

  • Good interpersonal and organizational skills.

  • Advocacy skills.

  • Proficient with Windows-based computer systems, in particular email and MS Word.

  • Available to work occasional evenings.

  • Ability to travel in a time-efficient manner to various locations within a reasonable distance of


Language designation

English, written and spoken.

French, written and spoken.
