About the Role
The Community Health Worker (CHW) provides community health services on an outreach basis to senior’s living in the community. The CHW will work closely with other members of an interdisciplinary team to support the care coordination or case management for clients, including providing health education, advocacy, practical assistance and support, access to other programs and services, and assisting clients and their caregivers to build support networks.
Requirements
Bachelor’s degree in social sciences or equivalent combination of training and experience.
Minimum two (2) years’ experience in a community-based health setting.
Minimum two (2) years’ experience working with seniors.
Knowledge of related community resources for seniors
Ability to assess situations and to respond appropriately
Interest in a variety of health issues and willingness to learn and teach others about them.
Organizational skills including time management
Ability to work flexible hours based on client need.
Ability to work collaboratively in a multi-disciplinary setting, with clients and other agencies who are involved with the care of the clients.
Sound judgement and strong problem solving skills.
Proficient with Windows-based computer systems, in particular, email, MS Word and client management systems
Fluency in English; other languages an asset
Desirable
Knowledge and understanding of the health system and community resources in Ottawa
Experience in the health and social services field.
Experience with outreach and/or home visiting
Language designation
English, written and spoken
