About the Role
The Registered Practical Nurse (RPN) works within the scope of the position to generally preserve and promote the health of clients in the community served by CCHC and specifically as part of the Urban Health Team, with a primary focus on supporting the New Canadians Clinic. Using the skills and knowledge of the nursing practice, and following the standards and guidelines of the profession, the RPN provides care to individuals, families and groups within CCHC, and in the community while on outreach. Nursing care is provided within the context of an inter-disciplinary primary care team embracing a client-centred service delivery model and a trauma-informed and harm reduction philosophy; recognizing that the social determinants of health affect health throughout a person’s life.
Requirements
Diploma of Registered Practical Nursing.
Possession of, or eligibility for, registration with the College of Nurses of Ontario.
Eligible for membership with the Registered Nurses’ Association of Ontario.
Minimum of 3 years of recent nursing experience related to the area of practice.
Strong clinical assessment skills.
Strong skills in mental health nursing.
Skills and experience working with a diverse population.
Ability to work independently and within a multi-disciplinary team.
Knowledge of health promotion principles.
Ability to apply trauma-informed and harm reduction concepts and philosophy in delivering service.
Demonstrated understanding of the health system and ability to provide guidance in navigating same
Demonstrated ability to prioritize and triage tasks and responsibilities effectively, including the flexibility to address unanticipated needs
Good problem solving skills.
Strong written and verbal communication skills.
Excellent interpersonal and organizational skills.
Advocacy skills.
Demonstrated proficiency in accurate charting and documentation
Demonstrated ability to model “client centred” service delivery
Proficient with the use of computers and various applications including Microsoft Office Suite, electronic medical records (EMR) and electronic record keeping tools. Availability to work occasional weekends or evenings
