Health Equity Questionnaire
What is the Health Equity Questionnaire?
We ask all clients a few, short, optional questions about who they are and their living situation. This includes questions about language, ethnicity, income, gender identity, and sexual orientation.
In 2025 we updated the questionnaire so the questions are clearer and easier to answer. Starting in spring 2026, new and current clients will be asked to complete the questionnaire. This helps us keep your information accurate and up to date.
Why are we asking these questions?
Your answers help us understand your needs and connect you with programs that may support you. When many clients share this information, we can also see where there are gaps in care.
For example, we can learn which communities are getting preventive care—like chronic disease screening, mental health care, and health promotion—and which groups may have trouble accessing these services. We can then adjust our programs to better support people who may be underserved.
Do I have to answer these questions?
No. Every question includes a “Prefer not to answer” option. You should only share what you feel comfortable sharing.
Your answers are private and optional. Choosing not to answer will not affect your access to care or services in any way.
Who will see my information?
Only the people involved in your care will see your information. It will become part of your medical record. Your information is confidential and protected by law. It is protected under the Ontario Personal Health Information Protection Act.
