About the Program

Your health. Your goals. Your plan.

Free, one-on-one support to help adults improve their health and prevent chronic disease.

What we offer:
- Health education
- Chronic disease risk screening (cancer, diabetes, blood pressure)
- Personalized health coaching (S.M.A.R.T. goals)
- Support connecting to doctors and community services
- Follow-up and ongoing support

Who we serve:
Adults 18+ from equity-deserving communities, including people without a family doctor, newcomers and refugees, Black, Indigenous and racialized communities, 2SLGBTQIA+ community members, people facing mental health or substance use challenges, street-involved people, people with disabilities, and low-income residents.

Additional Details

Preventive Care Program participants will receive the following services and supports:

  • Health education about chronic disease risk factors (e.g., commercial tobacco use, alcohol use, physical activity and diet).

  • Risk assessments to identify and learn how to reduce risk for chronic disease (e.g., cancer, diabetes, hypertension), including assessing risk for social conditions that may impact health outcomes (e.g., food insecurity, social isolation).

  • Health coaching to collaboratively develop and document a preventive care plan, which includes their preventive care goals and action plans to make changes that support their health.

  • System navigation and relevant connections to primary care, resources, programs and services based on preventive care goals, including referrals or connections to community and social services (e.g., food banks, housing supports).

  • Monitoring through check-in conversations to support progress and motivation, as well as identify and address barriers early.
